Dear Member,
Welcome to a transformative journey in wellness optimization. It is our honor to serve you through the Compassion Center and the Center for Incubation & Findings Research (CIFR), specifically through the Center for Ehlers-Danlos Syndrome & Hypermobility Disorders. This program was created by patients, for patients, in the name of patients who live with EDS, HSD, and other categorically complex conditions.
We know you've been through a lot. And we know how exhausting it is to explain yourself, your body, and your experiences over and over again. This program is here to change that narrative.
Our aim? To shift the focus away from disease and symptom management and towards true wellness optimization. We believe in empowering your higher self through movement, nutrition, mindfulness, purpose, and personalized support. You are not a diagnosis. You are a whole person with a powerful story, and we are here to support that journey.
State of Residency: Why It Matters
Your state of residency determines what programs you're eligible for and which providers you'll work with. It usually aligns with your driver’s license, auto insurance, and primary residence. This helps us build a coordinated care team and ensure consistency across your wellness journey.

Registration Steps
- Start Registration: Complete the Web Registration Form (non-HIPAA). This collects your contact info and consent to communicate.
- Make Your Payment: Use our secure Stripe Payment Portal to pay your consultation fee. Debit/credit cards only.
- Complete Your Health History Form: Submit current diagnoses, two years of medical history (new patients), all medications and supplements, and supporting documentation via secure HIPAA form.
- Book Your Appointment: Choose your preferred date/time and check your email for confirmation and Zoom link. Join 15 minutes early.
Additional Information
- New patients: Provide two years of medical history.
- Returning patients: Submit recent records (past year).
- All patients: Submit releases and records before appointment.
- Veterans: You’ll receive a VA-specific release form via DocuSign. Return it or send records 14+ days prior to your appointment.

Fees & Policies
- Standard Fee: $299.99 USD (non-refundable). Sliding scale available.
- Chargebacks: $75 fee if service was rendered.
- Late Cancellation: $50 fee if under 24 hours notice.
- Refund Exception: Applies only to the EDS/HSD Program if all terms are met.
Note: Registering with Compassion Center does not register you with your state’s cannabis program.
Privacy Policy
We may use your health info for treatment, payment, and operations. Shared only with referred providers. Authorization is required for other uses.

Disclosures Requiring Authorization
All other uses require written authorization. You may revoke in writing.
Specially Protected Health Info
Additional protections apply to AIDS/HIV, mental health, substance use, developmental disabilities, and genetics.
Refund & Chargeback Policy
- Cancellation Policy: Missed appointments or late cancellations incur a $50 fee.
- No Refund Policy: All services are non-refundable once provided. EDS/HSD exceptions apply if terms are met.
- Returned Payments: Chargebacks incur a $75 fee. Please contact us before disputing charges.
